Highlights

  • The NHS is required to commission an independent investigation for significant serious incidents and the wider NHS is required under the Serious Incident Framework to investigate all patient safety/serious incidents. It may require additional expertise and independent scrutiny for an investigation. This framework supports such investigations.

Why this framework matters to you

The NHS is committed to learning from patient safety incidents to improve patient care. As such, this framework supports both NHSE and the wider NHS to compliantly commission Independent Investigations to capture insight and to inform improvement through investigation and exploration of the care, treatment and healthcare systems and processes for one or more patients at any level of the healthcare system.

The framework was conducted under PCR-2015 LTR and will be reopened and refreshed on an annual basis.
